     1.  In the PDF Test Column, click on EXAM and print all pages.

     2.  In the PDF Course Column, click on COURSE; this is your reading material.

     3.  Complete your Answer Sheet and fill out your Certificate of Completion where indicated with a Red X.

     4.  Mail your completed Answer Sheet, Certificate of Completion and payment to:
