address. If you have an account, click LOGIN to access your account. You MUST select AOC FastCheck,
then Add a New Batch; under Category, select Licensing; under Group, select Dept of Insurance DOI, then
Reason, select Licensing. Follow directions to complete your request to ensure the Department of Insurance
receives an official copy of your report.
> The report is valid for 60 days. Should you have questions regarding the above, please call 800-928-6381.
Please note: If you do not complete these steps, the DOI will not receive the official copy of your background check and you will need to re-apply for a new report.
Step 2: Please click on this link to begin the application process https://pdb.nipr.com/erl/start.jsp
Step 3: Click on this link to setup your e-Services Account - https://insurance.ky.gov/Eservices/default.aspx
Step 4: Schedule your exam using your E-services account. (Only after the background check and application have been
processed by the KYDOI)
NOTE: The first four steps will allow you to schedule and take the state exam. To obtain your license you must also have filed your financial responsibility forms. Whomever you represent as an agent should have this information. If you are not being appointed by an insurance company, call the Independent Agents of Louisville at 502-459-8880 Ext 32 and talk to Sophia Crowe. You will have to obtain a Surety Bond or an Errors and Omissions insurance policy. The only other item that may need to be filed is Form 8302. This is an appointment form that your insurance company will need to send in for you to sell insurance for said insurance company.
